1. How did you first become acquainted with the Church of Scientology?

3. Were there problems in your life that you thought scientology would address?

4. Did you see, experience, or hear about things that didn't seem right while you were in the Church of Scientology? What were they, and what convinced you to set aside your feelings?

5. Why did you choose to stay in the Church of Scientology?

6. Were you staff or public? If staff, was it at a mission or an org? Were you ever in the Sea Org or OSA? Which unit? If not on staff, did you ever volunteer to 'help out'?

7. Why did you leave the Church of Scientology? Was there a "final straw"?
You Know You're a Scientologist When:

1) The saying, 'If it sounds too good to be true, it probably is', never applies to Scientology.
2) You didn't know that all Scientologists have no idea who they were last lifetime.
3) You just spent $500,000 on Scientology services and now know with certainty that Hubbard's unproven theory that man is a spiritual being is in fact a theory, and it's unproven!
4) You just finished all 3 L's and OT 8 and still can't tell the difference between exterior with full perception and interior with full perception. Oddly your perception seems the same as it was before you joined Scientology.
5) You can give unbelievable details of the spaceship they were flying 5 billion years ago but asked what model car you drove last lifetime and you're lost.
6) You know that all the hard to believe success stories you read are all true. Like the one where the OT used his powers to make the hurricane go around his city and hit another, where it killed 135 people.
7) You know that no Scientologist has ever embellished their success stories.
8) You're sent to ethics after telling the registrar you have $1,000,000 to donate if you can gather at least 100 verifiable facts about your immediate past life.
9) You get very excited when you realize exactly who you were a billion years ago but never find it odd that you don't have a clue as to who you were last lifetime.
10) You know you'll achieve exactly what you were hoping to achieve when you first began Scientology, as soon as the church releases the next OT level.
11) Keeping your family together is far less important than it use to be. If the church says disconnect from a family member, you disconnect and maybe even attack.
12) You never find it odd that most of the OT 8's you know never talk about their wins or gains.
13) You're pretty sure you're very gullible.


8. Do you think the Church of Scientology needs to change some of its practices? If so, what should be changed? How did those practices affect your life?

10. Any additional comments you would like to make?
Two key facts the Church of Scientology does not want you to know:
(1) No Scientologist has ever or will ever be able to give you verifiable details about any past life.
(2) No Scientologist has ever been able to or will ever be able to, perceive outside of their body.

Therefore:
Hubbard's datum that man is a spiritual being is in fact an unproven theory.

Spend at least $300,000 and you'll KNOW WITH CERTAINTY it's an unproven theory.
